Illumina Library Preparation and Sequencing Submission

Please read carefully the instructions for sample submission for Illumina library preparation and/or Illumina sequencing, then use this web form to submit the appropriate submission form and other required files. If you are submitting samples for library preparation and sequencing on any Illumina platform (MiSeq, Nextseq, NovaSeq), you need to fill in the Illumina Library Preparation Form. If you have user-prepared libraries, you need to fill in the Illumina Sequencing Submission form.

Download the Illumina Illumina Library Preparation Form or the Illumina Sequencing Submission Form, and fill it in completely. Submission requests will be delayed if your form is not complete.

  2. Upload the your filled in Sample Submission Form
  3. Upload the Sample gel image, or Bioanalyzer traces,  or other required files.
